Pygeum: Human Evidence, Extracts, and Label Guide

Learn how to distinguish Prunus africana bark powder from specific extracts, read standardization and serving details, and understand what older human studies do and do not show for urinary symptoms.

Pygeum is a name used for African cherry bark preparations

Prunus africana is the accepted botanical name for an African montane tree in the rose family. Pygeum africanum is an older botanical synonym that remains common on supplement labels. Products marketed as Pygeum are usually made from bark, not from the fruit. 1

Brands mainly market Pygeum for prostate or male urinary support. The name alone does not identify whether a product contains raw bark powder, a concentrated bark extract, a specific lipophilic extract, or a small amount inside a multi-ingredient blend.

Identity begins with three details

Botanical species
Look for Prunus africana. Pygeum or African cherry without a botanical name gives less identity information.
Plant part
Human research and regulatory monographs concern bark preparations. Leaf, root, fruit, and generic whole-plant wording should not be assumed equivalent.
Preparation
Bark powder and extracts have different material bases. The amount of one cannot be converted into the other from the plant name alone.

Older trials found symptom signals, but the evidence has important limits

An older Cochrane review included 18 randomized trials with 1,562 men who had lower urinary tract symptoms attributed to benign prostatic hyperplasia. Some pooled outcomes favored Pygeum for overall symptoms, urinary flow, residual urine, or nocturia. However, the trials were small, lasted only 30 to 122 days, used different doses and preparations, and often did not use a validated modern symptom score. 2

Cochrane now labels its public summary of this review as historical and outdated. The National Center for Complementary and Integrative Health describes the evidence as limited because studies were short, small, and inconsistent in their methods and formulations. 3 4

A later randomized study compared 50 mg twice daily with 100 mg once daily for two months, followed by an open-label extension. It found similar results between the two schedules, but it did not include a placebo group in the randomized comparison. It cannot establish how much benefit came from the extract itself. 5

What can be concluded

Evidence-aligned
Specific Pygeum extracts produced short-term signals in some urinary symptom and flow outcomes in older studies.
Still uncertain
The size and durability of benefit with modern supplements, and whether different extracts perform similarly, remain uncertain.
Not established
The trials do not prove that a generic supplement prevents disease progression, avoids complications, or replaces medical evaluation and established treatment.

Clinical guidance is more conservative than supplement marketing. NICE advises against offering phytotherapy for male lower urinary tract symptoms because reliable evidence about effectiveness and safety is insufficient. 6

Symptom relief is not the same as shrinking the prostate

Claim: Pygeum is a prostate shrinker

Too strong. The clinical evidence centers on urinary symptoms and flow measures. It does not establish that a generic Pygeum supplement reliably reduces prostate size.

Claim: It physically de-bulks the gland

Not demonstrated in people. Cell and animal findings about inflammation, growth factors, or androgen-related pathways cannot be turned into a guaranteed human outcome. 7

Claim: It increases pre-ejaculate or semen volume

Not established for healthy users. Some older studies in men with BPH reported spermogram or prostatic-secretion measures, but the evidence does not establish that Pygeum reliably increases semen volume or pre-ejaculate volume in healthy users. 4

Claim: It is a perfect partner for saw palmetto

Not established. Two botanicals appearing in the same formula does not prove synergy, safety, or suitability with medicines and health conditions.

Urinary frequency, weak flow, nocturia, incomplete emptying, and urgency have multiple possible causes. A supplement claim cannot diagnose benign prostatic hyperplasia or rule out infection, obstruction, medicine effects, or another condition.

Bark powder and a standardized extract are not dose-equivalent

Four common label patterns

Pygeum bark powder
The milligrams describe raw or milled bark. They do not reveal the amount of an extract or a measured phytosterol fraction.
Pygeum bark extract
Look for the actual extract mass, extraction ratio or solvent when stated, and any separately declared marker.
Standardized extract
A percentage should name the marker it applies to, such as total phytosterols. Standardization improves disclosure but does not prove that the finished product works.
Prostate support blend
A blend may contain Pygeum beside saw palmetto, pumpkin seed, nettle, zinc, or other ingredients. A single blend total does not disclose the Pygeum amount.

Some regulatory product contexts use standardized extracts in a 12 to 14 percent phytosterol range, but this does not make one percentage a universal requirement for every product worldwide. A missing 13 percent claim does not prove that a product is fake, and a printed 13 percent claim does not verify identity or laboratory content. 8

Read the material first, then the milligrams

Seven label details worth checking

1. Botanical identity
Prefer a clear Prunus africana declaration rather than a marketing name alone.
2. Bark plant part
Confirm that bark is stated when the product is being compared with the historical clinical evidence.
3. Powder or extract
Preserve the exact preparation. Do not compare raw bark powder milligrams directly with concentrated extract milligrams.
4. Actual amount per full serving
Read the serving size and capsules per serving together. Separate the extract mass from any larger raw-bark-equivalent figure.
5. Standardization detail
Check whether the percentage names total phytosterols, sterols, another marker, or nothing at all. Keep the marker tied to the correct ingredient line.
6. Blend transparency
A proprietary-blend total cannot reveal the individual Pygeum amount or preparation. Learn how proprietary blends work.
7. Sourcing documentation
Country of origin, traceability, and certification details can add useful context. Marketing words alone do not verify legal or sustainable harvest.

The same front-label promise can hide three different materials

These fictional examples show how disclosure changes the interpretation. They are not product recommendations and do not predict a health result.

Three prostate-support labels

Label A: Prunus africana bark extract 100 mg, standardized to 13% total phytosterols
The extract mass and marker percentage are disclosed. If the statement is accurate and the percentage applies to the full extract mass, it describes 13 mg of total phytosterols. It does not verify contents, efficacy, or sourcing.
Label B: Pygeum africanum bark powder 500 mg
The printed number is larger, but raw bark powder is not automatically equivalent to a standardized lipophilic extract used in a study.
Label C: Prostate Support Blend 750 mg
Pygeum, saw palmetto, and stinging nettle are listed, but the blend total hides the Pygeum amount, preparation, and any standardization.

Takeaway: The largest number is not automatically the most study-relevant disclosure. Compare the material, actual extract amount, marker, full serving, and blend transparency.

Study amounts do not create one universal consumer dose

The older trials in the Cochrane review used 75 to 200 mg per day of Pygeum extracts, with 100 mg per day used most often. These figures describe specific research preparations and populations, not a general dose for every bark powder, extract, blend, or person. 2

The EMA monograph describes 50 mg twice daily for one specific soft bark extract in a traditional-use context. That preparation is not interchangeable with an unspecified supplement or raw bark powder. 9

Three dose traps

Raw-bark equivalent
A 10:1 extract may print a large equivalent amount. That is not the actual extract mass and does not establish clinical equivalence.
Blend total
A formula can exceed 1,000 mg while containing an unknown amount of Pygeum.
No established upper limit
There is no authoritative tolerable upper intake level or Daily Value for Pygeum. More milligrams should not be treated as automatically better or safer.

CITES-listed does not mean every Pygeum product is illegal

Kew reports Prunus africana as Vulnerable under the IUCN category data it displays. The species is also listed in CITES Appendix II, which regulates international trade so that trade does not threaten the species’ survival. 1 10

Appendix II is not a declaration that every product is illegally harvested, and Vulnerable is not the same category as Endangered. Responsible sourcing matters, but a consumer label cannot by itself authenticate origin, permits, quotas, plantation status, or harvest practices.

Useful sourcing information

Specific origin
A named country and supplier provide more traceability than a generic African-sourced claim.
Verifiable documentation
A certification or traceability claim is more useful when the brand identifies the standard, certifier, or documentation that supports it.
Careful interpretation
Sustainable, plantation-grown, or CITES-compliant wording is not independently verified merely because it appears on the package.

Short trials cannot establish complete long-term safety

Pygeum extracts were generally reported as well tolerated in the older short-term trials. Digestive effects such as nausea, constipation, or diarrhea were the most consistently reported; headache has also been described. This does not rule out rare effects or establish safety across all extracts, doses, and users. 4 9

Practical safety boundaries

Urinary warning signs
Fever, blood in urine, painful urination, spasms, worsening symptoms, or inability to urinate require prompt medical assessment.
Medicines and procedures
Do not replace prescribed treatment, cancel planned follow-up, or assume compatibility from an ingredient page. Clinical interaction evidence is incomplete.
Population limits
The human evidence mainly concerns adult men with urinary symptoms attributed to BPH. It does not establish routine use in women, adolescents, pregnancy, or breastfeeding.
Allergy and tolerance
Avoid the ingredient after a known allergic reaction, and stop use and seek advice for concerning symptoms.

New or worsening urinary symptoms should be evaluated rather than self-diagnosed as BPH from age, marketing copy, or a supplement label.
